Bring Your Ontario Health Card
To better serve you, Services de santé de Chapleau Health Services requires you to show your Ontario Health Card each time you come to receive care or services. Your Health Card number is the best way we have to identify you and to make sure we have access to all of your necessary information. Your Health Card number is also used to bill OHIP for some services provided.
Smoking
As per legislation, no smoking is permitted in the hospital or within 9 meters of an entrance. Cigarettes are not to be disposed of on the ground. Smoking is allowed in designated areas only, using designated safety ashtrays.
SSCHS Respects your Privacy
All information related to an individual at SSCHS is treated as confidential. This information may be written, verbal or in any other form. Even the knowledge that an individual is in the hospital must not be volunteered or acknowledged by staff without the consent of the individual.
Confidentiality extends to everything the staff at SSCHS learns in the exercise of their duties. It extends to both important and unimportant information, the nature of the patient/client's illness, the cause, the treatment and all information the patient/client may disclose with a view to giving a better understanding for treatment of the condition, etc.; all information the physician learns from the examination of the patient; the reactions of the patient, their conduct in the hospital; anything that may be disclosed while in delirium; their financial status; the conditions at home; or domestic difficulties. These are all confidential matters.
